Recommendation for Reappointment, Promotion, or Tenure Action
The University's latest revised form is available on the web. This form can be downloaded in its entirety but data cannot be inserted without the form completely readjusting. The other option is a download page by page to insert data. DECS has set up a pdf form in which data can be inserted without readjustment. The instructions for this follows.
Instructions:
Start by opening the link to the FORM D PDF below. Adobe Acrobat 8 or above are required and have been tested.
At the top of this form there is an option to "Save Data", please test this after filling in several lines of data to become familiar with the process. Please note that this works with Windows operating systems and has not been tested with Macintosh or Unix.
The browsers Firefox 2 & 3 as well as Internet Explorer 7 are supported. IE 6 should work as well, but has not been heavily tested. Contact DECS Support office if you need an upgrade.
After clicking "Save Data", a "Save" dialog box should pop up asking you to save this file to your computer.
Opening Your Saved Data
After saving this file, you will be able to double click on it from its location. The filename will have an .xfdf extension. After double clicking on the .xfdf file, Acrobat may pop-up a security message. Click "Allow".
